1. INSTRUCTIONS:   This is your booking request form.  Please mark the show(s) you are interested in booking, with the date requested for each show. You can request more than one show for a single performer.       
2. If you want to select additional shows from a different performer, YOU MUST FILL OUT A SEPARATE REQUEST FORM FOR EACH PERFORMER.     
3. you are unsure of the date, please indicate an estimated date/month in the message field and enter the first date (i.e. December 1, 2021) of the month estimated in the date field on this form.     
4. NOTE: Shows cannot be guaranteed available on any set date until booked.    
5. ALL PAYMENTS MUST BE RECEIVED PRIOR TO THE SHOW DATE.  Electronic Links for Payment will be sent which can be processed online.  Special arrangements for check receipt can be made but will incur a $10.00 processing fee.
